docs(contributing): define policy for fully automated contributions

Summary / 摘要

本 PR 更新了项目中关于完全自动化贡献的政策。

• 新增：在 AGENTS.md 中增加针对AI的指导，说明维护者有概率封禁执行完全自动化贡献的用户。

• 新增：在 CONTRIBUTING.md 中明确关于完全自动化贡献的政策，包括维护者在判断某账户是否主要通过自动化流程操作时可考量的因素。

• 注意：经由有意义的人工审核、人工交互(人工参与 (meaningful human involvement))的AI辅助贡献，不被视为完全自动化贡献。

• 修复：要求 Pull Request 描述必须遵循仓库官方的 PR 模板，不得更改其结构或必填部分。
