Docs: add enterprise adoption patterns and AGENTS.md anti-patterns

Proposal

Add an "Enterprise Adoption Patterns" section to the main README with practical examples for teams that need predictable agent behavior across many repos.

Candidate subsections

• Minimal AGENTS.md baseline for org-wide consistency
• Safe override patterns by subdirectory
• Common anti-patterns (overly long files, conflicting directives, stale project metadata)
• Suggested review cadence for AGENTS.md changes

Why this could help

Many teams now adopt AGENTS.md as governance infrastructure, not just repo metadata. A short enterprise section would make adoption more reliable and reduce misuse.

I can open a docs PR with a first draft if this aligns with maintainers' direction.
