Fixes #2623.

Every Claude picture lookup searched its transcript from the last /clear to the end for the record its ref names. A tool's picture then searched again, from the floor up to that record, for the call it answers. The lookups of one transcript take turns, so a page of screenshots read a long transcript up to twice per picture, one after another. This PR targets main as it is and doesn't depend on #2610.

  • Picture index: the boundary scan, which every lookup already runs and caches, now notes where each user record holding a picture starts (PictureIndex, notePicture). It does that for the first such record since the /clear with that uuid. A lookup reads that one record and judges it exactly as before.

  • Still searched as on main, so the first record that qualifies still decides:

    • a uuid seen on an earlier record, followed by the branch or not (loose);
    • any record after one too long for the scan to read (blind);
    • any record after a record the endpoint may not judge (unread, below);
    • any record after the loose cap (below);
    • the final unterminated line;
    • a record a rewind left behind (hidden).
  • Records the endpoint may not judge: the scan notes each record since the /clear whose shape could pass SHAPE_MAX_BYTES (a bound on its strings that never undercounts, following recordShapeSteps' escapes), or that is long enough to hold a call id callIdSteps doesn't read (mayBeUnread). Only lines over about 3.9 MiB are looked at, by indexOf jumps; a screenshot's single long string never qualifies. A picture after such a record on the branch is searched for, and a call before one is looked for forward from the floor (answersDrawnCallForwardSteps, main's search), so main's refusal holds.

  • A kept scan's bytes are checked before its index is trusted: a kept scan is checked by main only by the bytes just before where it ended, so an in-place rewrite of an earlier line, followed by an append, can keep it. The scan now also digests the bytes its index was built from (PictureBytes: chained xxHash3 over 64 KiB pieces at fixed offsets from the floor, digested from the windows the scan already reads, so no extra reads). A lookup trusts the index only for a file version (dev, ino, size, mtime, ctime) that a read of those bytes vouched for (vouchedPictureSteps):

    • a read that began after the lookup began, such as the lookup's own fresh scan, or another lookup's check; or
    • a read that began at least 3 s after the version's mtime and ctime, when the ctime has a fraction of a second (fineTimes), so the filesystem's times tick finer than that (PICTURE_RACY_MS, the racy-clean rule of RACY_MS in claude-command-history from fix(claude): recheck a same-size history rewrite within one timestamp tick #2620, for a same-size rewrite within one timestamp tick).

Otherwise the lookup reads the bytes once, in yielding steps, and lookups of the same version that began before that read share it, so a page pays one read per change. A lookup that began after a read, in flight or done, never shares it: it reads the bytes itself. On a source of whole-second times (sshfs/SFTP, ext3, HFS+, FAT), no read vouches for a later lookup, so every warm lookup there pays one read of the bytes. Bytes that differ turn the index off (forgetPictures) and the search decides. This covers the indexed record too, so round 2's line-boundary and record-digest checks are gone.

  • A picture the index holds no record of, in a version nothing vouched for yet, runs exactly as with the index off: the search for its record and main's forward call search, with no use of unread, loose or blind, and no read of the bytes on top.

  • loose is bounded: uuids of records the branch doesn't follow are kept as 32-bit marks (xxHash32), from the last /clear on. The scan appends marks to a flat log; a picture's lookup checks a table, then the log, and the log is folded into the table once lookups have read it 64 times over. Two uuids sharing a mark read as seen, which only sends that lookup to the search. Cap: MARKS_LIMIT is 128 Ki marks since the /clear (a 512 KiB log, or a 1 MiB table at most half full). Past it the marks are freed, no uuid is hashed, no later record is indexed, and every picture after that point is searched for, as on main.

  • Call search goes backward: from the result one window at a time, starting at 64 KiB and doubling up to IMAGE_SEARCH_WINDOW. The last block before the result that names the call decides, as the forward search did. A window starts at the start of the line its edge falls in, found by reading back 64 KiB at a time.

  • Event loop: each read of the back-walk and of the bytes check, and the record read after the boundary scan's last window, is a step of its own. A lookup still yields between bounded reads, as since fix(conversation): yield during Claude image scans #2604.

Where a kept scan's own floor, branch or hidden ranges are stale after an in-place rewrite, the answer is main's, since the search uses the same kept scan. Main keeps that blind spot (write a /clear in after a picture, keep the tail, grow the file, and main still serves the picture), and this PR doesn't change it.

Not changed: a key spelled "uuid".

Timing

Main 05c9133 against this PR, 7 interleaved runs each, a fresh process per run with the page cache warm, median [min–max], ms. Synthetic fixtures: big, 102 MB with 200 pictures of about 147 KB, half a user's and half a tool's; clear, 106 MB with a /clear at about 90 MB and 40 pictures after it; side, 96 MB of 500,000 sidechain records and a picture. Append rows model a live session: each process works on its own copy and appends a line before each timed lookup, so every timed lookup runs in the timestamp tick of the last write and pays the bytes check. The host also runs a VM using about 8.5 cores, so treat differences under ~15% as noise.

Round 4 (1b61272) changes only when a read vouches; interleaved against ec3b92b on the append rows it costs the same (big append page 610 against 612 ms, clear 714 against 729 ms, one bytes read per page on both). Rows marked * are from 664db03, the head before the last commit. That commit changes only pictures the index holds no record of and scans past the cap, which these fixtures' rows don't reach; the big append rows, re-timed on ec3b92b, moved with main's (+5–10% on both).

Warm history pages are 0.1 ms on both.

  • Cost of the check: reading a version's bytes again is about 30 ms wall and 40 ms CPU per 100 MB. That is what a lookup right after an append gives back of the warm speedup: a big user picture goes from 24 ms warm to 54–64 ms right after an append, against main's 100–104 ms. A page pays it once (392 → 438–488 ms, against main's 3.3–3.6 s). In a session quiet for 3 s, lookups don't pay it.
  • Side is past the cap: 500,000 sidechain records is more than 128 Ki marks, so the side picture is searched for as on main, and its warm lookups are back to main's cost (round 2 had them at 1.8 ms). The other side rows are within noise of main. In this run, cold CPU is +8% and the cold history page +13%, with overlapping ranges. A second interleaved run of 7 on the same trees gave the PR 1057 against 1086 ms cold CPU and 704 against 722 ms for the history page, slightly under main. By ablation in that run, what remains over main is the scan's digest and the first 128 Ki marks, a few percent at most.
  • History pages: big and clear are within 2–3% of main (the scan's digest). Side's +16% from round 2 is gone, as above.
  • A 24-picture page in a fresh process takes 6.5–10 s on both versions. Every lookup that starts before the first boundary scan is published runs a scan of its own. It behaves the same on main, and this PR doesn't change it.

Memory at 2M sidechain records (384 MB file), each version in its own process with MIMALLOC_PURGE_DELAY=0 bun --smol, one cold lookup and 5 warm, then Bun.gc(true); three runs, MiB. RSS before the first lookup is about 46 MiB on all three. Past the cap nothing is allocated, so 664db03's numbers stand for the head.

Bytes read per warm lookup, on a synthetic 40 MiB transcript (counting fs.readSync; measured in round 2, and a warm lookup in a vouched version reads the same):

tool picture, call far back, 1025 KiB lines 78.08 MiB 44.48 MiB

#2623 has the field numbers that prompted this: on a 1.7 GB transcript, the median image request went from 48 s to 0.7 s after a build with this change. The caveats are in that issue: that build also carried other changes, and the log has no timestamps.

Validation

  • Toolchain: Bun 1.4.2.

  • bun run check fast on 1b61272: unit: 10721 pass, 0 fail, 41 skip in 662 files. Workflow syntax, generated types and themes, typecheck and build all ok.

  • server/claude-images.test.ts and server/conversation*.test.ts: 264 pass, 0 fail. The image test file passes 43 of 43 with real main's module as the reference.

  • Compared with main: both compares three lookups: cooperative, synchronous, and the search alone (pictureIndexForTests(false): the index off and the forward call search, which is main's lookup code, on the same scan).

  • Repros for the round-3 case, through earlierRewritten: warm a lookup of B, rewrite an earlier line in place at the same length (more than 64 KiB before the end, keeping B's record and the tail), append, look B up again. The earlier line becomes (1) a record of B's uuid with picture A: main and the PR serve A; (2) an unreadable line naming the uuid: both refuse; (3) an unreadable line naming the call before its call record: both refuse; (4) as (3), for a tool picture past the cap: both refuse. On a117343, (1)–(3) serve B. Also: a same-size rewrite that keeps the file's times (one timestamp tick); a page right after an append, which reads the transcript once; times held on a 10 s tick, whole or with only the ctime whole, with B warmed 5 s in and an earlier line rewritten into A (both serve A); a rewrite landing right after a shared check read its window, with a second lookup starting then (B, then A, as main); and the ctime half of the 3 s rule (mtime an hour old, ctime 1 s old, as cp -p leaves it).

  • Cap test: 128 Ki sidechain records, then a sidechain uuid reused by a picture, then a fresh picture. The reused uuid gives null, as on main; the fresh picture is served, and its warm lookup and its lookup after an append each read one search's worth (under 1.5× the file); a picture before the cap reads under 1 MiB.

  • Seeded fuzz test: seed 2627, 200 transcripts, 3 steps of 8 lookups each, every lookup compared with the search alone. The transcripts hold duplicate, sidechain and isMeta uuids, records written twice with different pictures, /clears and rewinds, numeric call ids, and unreadable lines naming a uuid or call id before what they name. Between lookups each is appended to or rewritten in place, keeping the size or growing it behind the tail a kept scan is checked by. New this round: an earlier line rewritten in place, at the same length, into another record of a picture's uuid, or into an unreadable line naming that uuid or its call; and half the rounds give every write its own mtime well in the past, so vouches carry over and the trust-without-reading path runs; and a fifth of the rounds report one whole second for every stat, held, as a coarse timestamp source would. With real main as the reference, seeds 2627 and 1–10, 400 transcripts each: no differences on this head, and differences from round 12 of seed 2627 on a117343, and at rounds 61 and 174 on ec3b92b.

  • pictureIndexForTests stays exported: conversation.ts already exports test hooks the same way (limitCodexHistoryForTests), as do codex-images.ts (blobPlatformForTests, aroundBlobOpenForTests) and others. I'm happy to move it if you'd prefer another mechanism.

  • Not run locally: the integration and browser suites, native Windows and macOS. Upstream CI covers them.

All fixtures are synthetic files in temporary directories. No real transcript, image or screenshot was used.

…he transcript

Each image lookup searched the transcript from the last /clear to its end
for the record its ref names, and a tool's picture searched again from the
floor for the call it answers. The lookups of one transcript take turns, so
a page of screenshots in a long session read the whole file twice for each
one, one after another.

The boundary scan, which every lookup already runs and keeps, now notes
where each user record that holds a picture starts, when it is the first
record since the /clear with that uuid. A lookup reads that record and
judges it as before. A uuid seen on an earlier record (one the branch
follows or one it does not), a record after one too long for the scan to
read, the final unterminated line and a record a rewind left behind are
still searched for, so the first record that qualifies still decides.

The search for a tool's call goes back from its result a window at a time.
The last block before the result that names the call decides whether the
result is drawn, as it did when the search went forward, so a call made just
before its result costs a window. A window starts at the start of the line
its edge falls in, found by reading back a short piece at a time, so a call
far back costs about one read of what lies between, where the forward search
read that and the whole transcript for the record. Each of its reads, and
the read of the record after the boundary scan's last window, starts a step
of its own, so a lookup still gives the event loop its turn between bounded
reads.

The tests that change the file during a lookup now trigger on the read of
the picture's record, which the search no longer reaches by reading the
transcript's first window.

Summary by CodeRabbit

  • Bug Fixes
    • Fixed slow image loading in long Claude conversations, especially for screenshots from tool interactions.
    • Improved image lookup accuracy when conversations include cleared or rewound history, and when matching tool calls to their results.

Claude picture lookups now use an index built during transcript boundary scans when the indexed record is valid. Tool-call lookups search backward from the result in widening spans. Tests cover read volume, matching behavior, and filesystem mutations.

Changes: Claude picture lookup

Layer / File(s) Summary
Index and retrieve picture records
server/conversation.ts, server/claude-images.test.ts, changelog.d/claude-image-lookup-index.md
The boundary scan indexes eligible picture records by UUID. It clears the index at /clear and stops adding entries after an oversized record. Picture lookup checks the index and falls back to transcript scanning when the entry is invalid. Tests exercise filesystem mutations, duplicate UUIDs, and /clear.
Search backward for tool calls
server/conversation.ts, server/claude-images.test.ts
answersDrawnCallSteps searches backward from the tool result, expanding its read span from 64 KiB to 1 MiB. Tests compare synchronous and asynchronous lookups and check read-volume bounds and call matching.

The optimization improves ordinary picture lookups, but unusual transcripts can retain expensive scans or select a different picture. The change is mergeable with awareness of these narrow limitations; the release notes should avoid promising universal improvements.

Explanation

Directly linked issue #2623 has coding requirements only. The diff to server/conversation.ts implements each proposed change:

  1. Picture index. The boundary scan calls notePicture for each branch record. It stores the first user record with a picture for each uuid since the last /clear (PictureIndex.at). holdsPicture checks image blocks and tool_result blocks that contain images. Indexed records are judged by indexedPicture, which returns undefined when the record lies outside the floor or stream, or overlaps stream.hidden.
  2. Fallbacks. Uuids seen earlier, on or off the branch (ends, loose), are not indexed. Records after a blind oversized record (blind) are not indexed. The unterminated last line is not indexed, because only newline-terminated records reach followClaudeBranch. /clear clears at and blind. When indexedPicture returns undefined, claudePictureSteps uses the original forward linesWithSteps search.
  3. Backward call search. answersDrawnCallSteps starts at CALL_SEARCH_START (64 KiB) and doubles up to IMAGE_SEARCH_WINDOW. Each window starts at a line boundary found by reading back 64 KiB at a time. Records are judged in reverse, and the latest matching call decides. Each read yields. The loop always makes progress, because from is always below to until the floor.
  4. Event loop. Yields follow each back-walk read and the read after the boundary scan.

The PR description and the summary list tests for warm read cost, near and far call search, duplicate uuids including sidechains, 12 and 20 MiB records, and /clear. I did not read the test file. I rely on the description and summary for those.

Unresolved points:

  • The index stores the first user record with any picture. The lookup judges only that record. I could not read the rest of claudePictureSteps in the truncated file. So I cannot confirm that a later duplicate-uuid record holding the referenced picture index is still reached when the first record lacks that index. This edge case is rare.
  • answersDrawnCallSteps returns the decision of the last matching block before a null call resolution. The forward search returned false on any earlier null. The new result can differ in that case. The new behavior is consistent with the latest-call rule, but it is not identical to the forward search that the PR describes.

These points do not show a missing objective. The issue objectives are implemented.

Explanation

All changed files relate to the Claude picture lookup path in #2623. server/conversation.ts adds the picture index, its fallbacks, the backward call search, and the yields. server/claude-images.test.ts adds tests for read cost, call matching, uuid selection, oversized records, /clear, and rewind. It also adjusts race-test triggers so they still fire under the new read pattern. changelog.d/claude-image-lookup-index.md adds a Fixed entry for the same change. No hunk changes unrelated behavior. The summary reports no changed declarations of exported entities.

Clear loose with the other picture state on /clear.

/clear resets at and blind but leaves loose. A post-clear picture record that reuses a UUID from an off-branch record before the clear is never indexed. Its lookups then search from the floor. Add loose.clear() to the reset.

Reuse the bytes the call-search walk already read.

When a tool result follows a multi-MiB single-line record, answersDrawnCallSteps walks back through that record in 64 KiB reads. linesWithSteps then reads the same bytes again in 1 MiB windows. A 20 MiB record costs about 340 reads and roughly twice its size in bytes. On a high-latency mount this can be slower than the previous forward scan.

The new index treats records not returned by followClaudeBranch as unseen. That follower skips a line lacking the literal "uuid" token, so an earlier valid JSON record with an escaped UUID key could be missed and a later record with the same UUID indexed as first.

The lookup may return the later picture although the prior first-record search would return no image.

This can occur when: An earlier record spells the key as "\u0075uid" and has no picture, followed by a conventional picture record with the same UUID after the history floor.

Thanks @duytbp, this is a nice cut of the per-picture cost, and it holds up well: omo's side-by-side fuzz (36,000 comparisons over duplicate/sidechain/isMeta uuids, /clear, rewinds, numeric ids, multi-byte padding) and 300 window-edge cases found no difference from main. Codex Terra xhigh and omo did reproduce a few cases where the answer differs from main, and the bar for picture lookup is the same answer (or the same miss) as main, so we'd like one round on this branch:

  1. Keep main's refusal when an earlier record can't be judged. If an earlier record that names the picture's uuid is too big for main to read its shape (e.g. a 4.6 MB progress or file-history record), main returns nothing; the indexed lookup (conversation.ts:2579) skips it and serves the later picture. The backward call search (:2480) does the same for an earlier unreadable record that names the call id. Repros: /tmp-style fixtures available on request; in short, an oversized line containing the uuid/call id before the real record. Please note such records in the scan (as blind does for the ones it can't read) and fall back to main's search, or refuse, when one precedes the indexed record or the call.
  2. Validate indexed ranges on line boundaries. The index trusts offsets from a reused scan, which is only checked by its last 64 bytes, so an in-place rewrite that grows the file between lookups keeps old offsets: with a duplicate uuid it served pic1 where main serves pic2, and a record shifted by 100 bytes returned nothing where main finds it (:403-405, :477, :2569-2573). Checking that the indexed range starts after a newline and ends on one, and otherwise searching as main does, covers the shift; the exact-alignment case needs a cheap identity check of the record (its uuid at the expected place).
  3. Bound PictureIndex.loose. It keeps every distinct unfollowed uuid, including sidechain records with no pictures, across /clear and into cooperative-read clones: an isolated 500,000-sidechain-record fixture went from 49 MiB to 154 MiB RSS after a lookup (main 53 MiB), with up to 32 scans cached. Please bound it (or store only what a lookup needs) and add a small memory table (main vs PR) for that fixture.
  4. Noted, no change needed: a key spelled "\u0075uid" makes the branch scanner skip the record and the PR pick a later picture; Claude never writes that.

Smaller: the test named for clear and rewind (claude-images.test.ts:462) contains no rewind, and both compares the PR's two readers rather than either with main.

After that round we'll run our own interleaved main-vs-PR timing (a single picture cold/warm, a 24-picture page, a long transcript after /clear, CPU per lookup); the merge needs every row better or the same and the wrong-image cases identical to main.

Thanks for the fast and thorough review. I'm working on all three in this PR:

  • I'll note earlier records that can't be judged, and refuse or fall back as main does, for both the picture's uuid and the call id.
  • I'll check indexed ranges against line boundaries and the record's uuid, and search as main does otherwise.
  • I'll bound loose, with a memory table for the 500,000-sidechain fixture against main.

I'll also fix the two tests: the rewind case, and comparing with main rather than between the PR's two readers. And I'll add a seeded fuzz test that compares every lookup with main's search.

The failing browser job is take-over-regression.ts:68. I've also seen it fail on a branch that doesn't touch server code, so I don't think it comes from this change.

…ch's answer

A record the endpoint cannot judge refuses the search for a picture or
a call it names, wherever it lies before the record or the call. The
index skipped such a record and the call search going back never
reached one before the call, so both served what the search refuses.
The boundary scan now notes each record since the /clear whose shape
could pass what the endpoint holds, by a bound on its strings, or that
is long enough to hold a string past what it reads whole. A picture
after one is searched for, and a call before one is looked for forward
from the floor, as the search does.

A kept scan is checked by the bytes before its end only, so a rewrite in
place that grows the file keeps the offsets it indexed. An indexed
record is now taken only while it still starts and ends a line and holds
the bytes the scan read there, as reading its shape digests them;
otherwise the search decides, and that scan's index is not used again
until a /clear starts another.

The uuids of records the branch does not follow are kept as 32-bit
marks in a table at most half full, from the last /clear on, instead of
every uuid whole: a mark two uuids share reads as seen, which only sends
a lookup to the search. A kept scan of 500,000 sidechain records holds
about 4 MiB of them instead of about 45.

The tests compare each lookup with the search alone, which is main's
lookup, and the test named for a rewind has one.
… random transcripts

A seeded run of 200 small transcripts, each looked up 24 times through
appends and rewrites in place, compares every lookup with the search
alone. The transcripts hold duplicate, sidechain and meta uuids,
/clears, rewinds, numeric call ids and records the endpoint cannot judge
before a picture or a call they name; the rewrites keep the size or grow
the file behind the tail a kept scan is checked by, and move, join,
drop or change records before it.
…look them up only for pictures

The boundary scan every Claude history page runs hashed each sidechain record's uuid twice and put
it in an open-addressing table, so a cold page of 500,000 sidechain records cost about 30% more CPU
than before the picture index. Now the scan hashes a uuid once, only for a record the branch does not
follow or one that holds a picture, and appends it to a flat log. A picture's record looks the log up
with includes, and the log is folded into the table once lookups have read it 64 times over, so a
file of many pictures does not read a long log for each.

Pushed a117343 (four new commits on aa57772, no rebase), and the description is updated. Taking the review in order:

  1. Earlier records that can't be judged: the scan now notes every record since the /clear whose shape could pass SHAPE_MAX_BYTES, or that could hold a call id too long to read. A picture after one is searched for as on main, and a call before one is looked for forward from the floor, with main's search. Both of your repros are tests now: an oversized line naming the uuid before the picture, and one naming the call id, before the call and between the call and its result.
  2. Indexed ranges: a record is taken only if it starts after a newline, ends on one, and its bytes match the digest the scan took. The digest is computed while the record is read anyway, so it costs no extra read. It also catches the exact-alignment case: a same-length record with the same uuid moved onto the offset, which a uuid check at the offset would pass. On any mismatch, main's search decides, and that kept scan's index isn't used again until a /clear. That last step came from a two-rewrite sequence the fuzz found against real main.
  3. loose: it's now 32-bit marks, reset at /clear. On the 500,000-sidechain fixture, RSS after warm lookups is 68–71 MiB, against main's 63–65 and 194–202 on aa57772. The JS heap is 6.9 MiB, against main's 3.0 and 47.6 before. The memory table is in the description.

The smaller items: the /clear test now has a rewind. both compares each lookup with the search alone, which is main's lookup code on the same scan. The whole test file also passes with real main's module as the reference. A seeded fuzz test compares every lookup with that search. The same generator against real main gave no differences in about 105,000 comparisons. CodeRabbit's changelog point is fixed too: the speed claim now covers only lookups the index answers.

I ran your timing rows ahead of your pass: main against a117343, 7 interleaved runs each, on synthetic 96–106 MB transcripts. Every picture-lookup row is better or the same, in wall time and in CPU:

  • a warm lookup takes 22–37 ms against 48–174 ms, and on the sidechain fixture about 2 ms against 405;
  • a warm 24-picture page takes 0.5 s against 4.3 s;
  • a first lookup, which includes the boundary scan, takes 326 against 469 ms.

One row is worse, and I'd ask you to accept it. History pages run the same boundary scan, so they now build the index without using it. On the 500,000-sidechain fixture, a cold history page costs about 16% more CPU (925 against 796 ms, overlapping ranges). The other two fixtures are within noise. An earlier version of this round cost +32%. Profiling put all of that in the uuid marks, and appending them to a flat log instead of a table cut it to the hash and the append. The same transcript's first picture lookup is about 25% faster. If you'd rather the history page match main exactly, I can build the index only on a picture lookup's scan. That costs a second scan whenever a history page came first, which is the usual order.

Also noted in the description: a 24-picture page in a fresh process takes 8–12 s on both main and the PR. That seems to be every lookup that starts before the first scan is published running a scan of its own. It's unchanged here, and could be a separate fix.

Thanks @duytbp, this round is solid. Merge-pass delta on a117343 (omo + Codex Terra xhigh):

Done and verified. Unjudgeable records before the picture or the call fall back to main's search (your repro tests fail on aa57772 and pass now, against main's real module); indexed ranges are taken only on line boundaries with a matching digest (the shift, exact-alignment and two-rewrite cases match main); the loose-uuid set is hashed and reset at /clear (RSS on a 500k-sidechain fixture is back to main's level: ~61-63 MiB vs 115-125 on aa57772). The seeded random-transcript comparison is real: pointed at main's own transcriptImageAsync it passes on a117343 and reports many differences on aa57772.

One case still differs from main (both reviewers reproduced it), which keeps it from meeting the bar (wrong-image behaviour identical to main for a rewrite between lookups): a reused scan is checked only by its last 64 bytes (conversation.ts:494), and the digest proves only the indexed record itself (:2738-2742). Warm a lookup for picture B, rewrite an EARLIER same-length line in place into picture A with the same uuid (or into an oversized record naming the uuid, or naming the call before its call record), keep B and the old tail, then append a line: main returns A (or refuses), the PR still returns B. Please make a reused scan notice an earlier in-place change, or fall back to the search whenever it can't rule one out, and add the repro as a test; if the cheapest sound check costs part of the warm speedup, say how much in the table and we'll weigh it.

Smaller points (not blocking): the loose-uuid log grows without a fixed cap between clears (Terra measured 8.4 MiB external at 2M sidechain records, vs 0.4 on main): a cap with a fallback to the search would close it. pictureIndexForTests is exported from production code; history pages build the index without using it (+16% CPU on a cold page per your table).

We'll re-run the ABBA table on your next head.

Addendum to the round above: please also give the loose-uuid log a fixed cap (past it, fall back to the search), with a measured memory table at 2M sidechain records (main vs your head, RSS and external/typed-array bytes). On the rewrite case: the matched record's own digest is already checked and still matches there, since the change is in an EARLIER line (a same-uuid record or an unreadable one now sits before it), so the fix needs to notice that earlier part of the scan changed, or fall back to the search when it can't rule that out.

Thanks. Working on both in this PR. The blocker comes first: your three earlier in-place rewrite repros as tests, then making a reused scan notice an earlier change or fall back to the search. The random-transcript test will generate that case too. The timing table will add a row for a lookup right after an append, so the cost of the check is visible. Then a fixed cap on the loose-uuid log with a search fallback, and a memory table at 2M sidechain records (RSS and external/array-buffer bytes, main against the head).

… picture index

A scan kept for a Claude transcript is checked only by the bytes just before where it ended. So an
in-place rewrite of an earlier line, to another record of a picture's uuid, or to one the endpoint
cannot judge naming that uuid or its call, followed by an append, kept the scan and its index, and
the lookup served the indexed record where the search would serve the other picture or refuse.

The scan now digests the bytes its index is built from, in 64 KiB pieces at fixed offsets. A lookup
trusts the index only for a file version a read of those bytes vouched for: one begun after the
lookup itself, or one begun at least 3 s after the version's mtime and ctime (the racy-clean rule
claude-command-history keeps for a same-size rewrite within one timestamp tick). Otherwise it reads
them once, and the lookups of that version begun before the read share it. Bytes that differ turn the
index off, and the search decides.

That check covers the indexed record too, so the line and digest checks of the record alone are gone.
The seeded comparison with the search now also rewrites an earlier line in place, at the same length,
and gives half its transcripts the times of a session long quiet.
The marks of records the branch does not follow grew with every sidechain record since a /clear. They
now stop at 128 Ki: a log of 512 KiB, or a table of 1 MiB at most half full. Past that point none is
kept and every uuid reads as seen, so each picture's record after it is searched for, as main searches
for every one.
…ord of without reading its bytes first

A lookup whose picture the picture index holds no record of uses the index
only for the call search. When no read has vouched for the file's version
yet, such a lookup now runs as with the index off (the search for its record
and the forward call search) instead of reading the index's bytes on top of
that, and it neither vouches for nor forgets the index. Past the uuid marks
limit the scan no longer hashes uuids, since no record is indexed then.

Pushed 08fce01, 664db03 and ec3b92b (fast-forward from a117343, no rebase), and the description is updated.

The rewrite case. A kept scan now digests the bytes its index was built from, from the floor to where it read (chained xxHash3 over 64 KiB pieces, taken from the windows it already reads). A lookup trusts the index only for a file version that a read of those bytes vouched for: one that began after the lookup did, or one that began at least 3 s after the version's mtime and ctime (#2620's racy-clean rule). Otherwise it reads the bytes once, shared by the lookups of that version, and on a mismatch the index is off and the search decides. That also covers the indexed record, so round 2's per-record line and digest checks are gone. Your three repros are tests: each fails on a117343 (serves B) and passes here, giving main's answer (A, or a refusal), with real main's module as the reference too. A fourth covers a tool picture past the cap. The fuzz test now generates earlier same-length rewrites into a same-uuid picture or an unreadable line naming the uuid or call. Against real main it reports differences on a117343 and none here (seeds 2627 and 1–10).

What the check costs, main 05c9133 against the head, 7 interleaved runs (the full table is in the description):

row main PR, warm PR, right after an append
big, user picture 100–104 ms 24 ms 54–64 ms
big, tool picture 136–147 ms 20 ms 47–52 ms
big, 24-picture page 3.3–3.6 s 392 ms 438–488 ms
clear, 24-picture page 1.0–1.2 s 557 ms 536 ms

One check is about 30 ms wall and 40 ms CPU per 100 MB, paid once per file version: a page shares it, and a session quiet for 3 s doesn't pay it.

Cap and memory. MARKS_LIMIT is 128 Ki marks since the /clear (512 KiB of log, or a 1 MiB table). Past it the marks are freed, nothing is hashed or indexed, and later pictures are searched for. At 2M sidechain records (three runs each, MiB):

version RSS JS heap external arrayBuffers
main 05c9133 70.3–74.2 3.0 1.3 0.0
this PR 73.8–75.1 3.1 1.4 0.0
a117343 75.5–76.0 10.9 9.2 8.0

The trade-off: the 500k-sidechain fixture is past the cap, so its warm lookups are back to main's ~350 ms (round 2 had 1.8 ms). Its other rows are within noise of main: one run shows the cold lookup and cold history page at +8% and +13% CPU with overlapping ranges, and a second run on the same trees shows both slightly under main. History pages on big and clear are within 2–3%, so round 2's +16% is gone.

If you want that fixture's speedup back, a fixed 1 MiB Bloom filter (k = 3) in place of the log and table would do it. A false positive only sends a lookup to the search, so it stays sound. It would give a search for about 0.44% of pictures at 500k marks, 2.7% at 1M and 13% at 2M. I haven't built it; I'd do it as a follow-up if you want it.

pictureIndexForTests: I kept it. conversation.ts already exports test hooks the same way (limitCodexHistoryForTests), as do codex-images.ts (blobPlatformForTests, aroundBlobOpenForTests) and others. I'm happy to move it if you'd prefer another mechanism.

Checks on ec3b92b: check fast 10717 pass, 0 fail; the image and conversation tests 260 of 260. Every new guard, undone alone, fails at least one test; the table is in the description.

Thanks @duytbp, this round closes both earlier points: the earlier in-place rewrite cases (same-uuid picture, unreadable record naming the uuid or the call, also past the cap) now match main, an mtime-preserving writer is still caught through ctime, and the cap holds (2M sidechain records: ~0.0-0.5 MiB extra after a lookup vs 8 MiB on a117343). The new tests and the fuzz fail on a117343 and pass here. omo approves.

Codex Terra reproduced two cases that still differ from main; by the pass's rule a reproduced wrong-picture case gets one more small round:

  1. A coarse timestamp source (conversation.ts:2793-2795): the 3 s quiet rule assumes file times tick finer than 3 s. With mtime and ctime held for a 10 s tick (mocked), warm picture B 5 s into the tick, rewrite an earlier same-length line to picture A: main returns A, the PR still B. Local filesystems tick finer, but NFS can cache file times for up to 60 s. A cheap guard: vouch only when the version's mtime/ctime show sub-second precision (non-zero nanoseconds), or the platform's timestamps are known fine; otherwise verify the bytes as for an unvouched version.
  2. A rewrite during the shared verifying read (:2817-2826): two concurrent lookups share one verification; with the timestamps unchanged, a rewrite landing right after the verifier read its window gives B, B where main gives B, A. Let a lookup join a verification only if it started before that verification began (otherwise run its own), or re-check the version after the shared read.

Please add both as tests. Smaller (not blocking): a test for the ctime half of the 3 s rule; forgetTranscriptState doesn't clear pictureChecks; the lower bound on bytes read at claude-images.test.ts:444 mirrors the implementation; vouches takes two shapes for one idea. We'll re-run the timing table on your next head.

Thanks. Working on both in this PR, each with a test. The racy vouch will count only when the version's mtime and ctime show sub-second precision; otherwise the lookup checks the bytes. A lookup will join or reuse a shared check only when that check can vouch for it, and otherwise check on its own. I'll do the smaller points too: a test for the ctime half, clearing pictureChecks in forgetTranscriptState, an outcome bound in place of the lower bytes bound, and one shape for vouches.

… file's times can date

The quiet rule (a read begun 3 s after a version's mtime and ctime vouches
for it to later lookups) assumes a write made after those times moves them.
A timestamp source of whole seconds, or one that holds times for a while,
breaks that, so the rule now applies only when the version's ctime has a
fraction of a second; otherwise a later lookup reads the bytes again.

A lookup now shares another's read of the index's bytes only when it began
before that read did. One that began after it may have stat'ed a version a
rewrite left with the same size and times, after the read took its bytes.

Also: forgetTranscriptState drops the shared reads, a read is one
{ began, wall } shape wherever a vouch is judged, a test covers the ctime
half of the quiet rule, and the page-after-append test no longer asserts a
lower bound on bytes read. The seeded comparison gains rounds whose stats
report one whole second, held.

Pushed 1b61272 (fast-forward from ec3b92b, no rebase), and the description is updated. Both cases are tests that fail on ec3b92b and pass here.

1. Coarse timestamps. The 3 s quiet rule now counts only when the version's ctime has a fraction of a second (fineTimes). I judge the ctime alone: every write sets it and no writer can, while a writer can give the mtime any fraction. Otherwise a later lookup reads the bytes, as for an unvouched version. The test holds the times on a 10 s tick, warms B 5 s in and rewrites an earlier line into A. It runs twice, with both times whole and with only the ctime whole, and both now serve A. The cost: on a whole-second source (sshfs/SFTP, ext3, HFS+, FAT), every warm lookup reads the index's bytes once, about 30 ms per 100 MB, and a page still shares that read.

2. A rewrite during the shared read. The joiner had begun after the check: the check began at 73.11 ms against its 73.77 ms. It still shared the read because vouches also let a shared check through by the quiet rule, so "began before me" and "quiet" were treated as the same thing. Now a check, in flight or done, is shared only by lookups that began before it, and any other lookup reads on its own. The quiet rule applies only to a vouch kept on the index. So sharing no longer depends on timestamps, since a joiner always gets bytes read after its own stat. Checking the version again after the read wouldn't have caught it, because the rewrite kept the times. The test gives B, B on ec3b92b and B, A here, which is main's answer.

What still depends on timestamps is the sequential case: a vouch, then a rewrite that keeps fine-grained times, then a later lookup. That needs a rewrite inside one ctime tick, which userspace can't arrange, so I kept the quiet carry rather than paying a bytes read on every warm lookup (a big user picture would go from about 24 ms to 55–65 ms; main is about 100). claude-command-history relies on the same RACY_MS rule.

Smaller:

  • ctime half: a new test with the mtime an hour old and the ctime 1 s old, as cp -p or rsync -t leaves a file.
  • forgetTranscriptState now clears pictureChecks. With the join rule, a kept check can't vouch for a later lookup anyway, so this only frees memory (at most 32 checks of up to 64 KiB). No test can see that without exporting another hook, so I didn't add one.
  • claude-images.test.ts:444: I dropped the lower bound and kept the upper one.
  • One shape: PictureRead { began, wall }, used for both the shared check and the vouch kept on the index.

The fuzz test has a new kind of round in which every stat reports the same whole second. Against ec3b92b it finds differences at rounds 61 and 174 of seed 2627. Against real main, seeds 2627 and 1–10 show no differences on this head. Each new guard, undone, fails at least one test. Interleaved with ec3b92b on the append rows, this round costs the same: a big 24-picture page right after an append takes 610 against 612 ms, with one bytes read per page on both. Checks on 1b61272: check fast 10721 pass, 0 fail; the image and conversation tests pass 264 of 264.

…not by scheduling

The in-flight change tests mutated the file after one setImmediate and assumed the lookup was still reading; on a fast machine it could finish first. A readSync spy now applies the change right after the lookup's first read of the file returns, and the test asserts it was applied.

Thanks @duytbp for this and for four careful rounds: a Claude picture's record is found through an index the boundary scan builds, instead of searching the transcript on every lookup. Merged on Dori's call.

Review. omo and Codex Terra (xhigh) reviewed every round. The last delta (1b61272) has no HIGH or MEDIUM under the bar for this PR (wrong-picture behaviour identical to main on a plain local disk with Claude's own clear/rewind/append): earlier in-place rewrites, unreadable records naming the uuid or call, stale prefixes, moves, joined lines, same-length replacements, /clear, rewind, the mark cap and 84 window-boundary cases all match main; the 2M-sidechain memory is 0.8 MiB retained vs main's 0.7.

Known limits (not blocking): a rewrite that lands while two lookups are already in flight, with the file times held fixed (mocked), can give the second one the pre-write picture (a later lookup gets the new one); an escaped \u0075uid JSON key, which Claude never writes, is read differently. The merge pass made the in-flight change tests deterministic (32b99ca: a readSync spy applies each change right after the lookup's first read, instead of relying on one setImmediate); 20/20 plain and 10/10 single-core-with-load runs pass.

Measured (merge pass, main 3cc5ced vs 1b61272, ABBA, 8 rounds, fresh process per run): every row better or the same, and main and the PR returned byte-identical pictures (or both null) for all 55 refs in 4 ways.

CodeRabbit's check shows skipping on this head; the merge pass and both reviewers reviewed it.
