Adding ejentum-mcp, an MCP server exposing the Reasoning Harness as four cognitive operating modes for Claude Code: harness_reasoning, harness_code, harness_anti_deception, harness_memory. Each tool call returns a structured scaffold (failure pattern, procedure, suppression vectors, falsification test) the model ingests before generating, addressing the four mechanism failures common in long Claude Code sessions: attention decay, reasoning decay, sycophantic collapse, hallucination drift.
