docs: describe the shipped /pool wire format (coords + axisIds) - #128

The latent-space design doc still said /pool sends {text, tier, x, y, z}
and that axes carry assignedTo: 'x'|'y'|'z'. Workstream C shipped a
variable-length coords array indexed by the ready axes, plus axisIds to
bind them. Record what shipped and why, keep the no-embeddings-on-the-wire
argument.
